Re: Addendun to Funeral Planning Worksheet.
My immediate family (parents and three sisters) are deceased. I have a nephew (Paul Antonello
in Skokie) and a niece (Mary Pawlowski in Arlington Heights). Other nieces and nephews live
far away across the United States. To inform my relatives of my death, please begin with Paul
Antonello (Cell: 847-274-0337). Tellhim first to phone my niece Carol Barker in North Carolina
(Cell: 503-867-4022). Ask him to have her call her siblings (my nieces and nephews-who live in
Califomia,Colorado, Michigan and Connecticut). Next, ask Paul to call my niece Mary Pawlowski
(Cell: 224-374-5779). Finally, would you please call two close friends of mine: Mrs. Eleanor
Buron who lives in Texas (Cell: 630-302-3953) and Dr. Elaine Lee, PhD in Northbrook (Cell:
847-370-1958). Thank you very much.
